Medical Records Requests

Pre-Hospital Care Reports are protected under Federal and State laws. To ensure that the Gilbert Fire & Rescue Department complies with its obligations, a JustFOIA request form must be filled out for each record requested along with the required information associated with your request.

Within the JustFOIA request form, you will be given a list of documents needed to fulfill your request based on the requestor’s relationship to the patient.

Service of legal documents, including Subpoenas for Records and liability claims, against the Town can be delivered in-person to Muni I located at 50 E Civic Center Drive.

FAQs

How long does it take to receive a medical request?

Once the form and all required documentation have been received, Gilbert Fire & Rescue will process the request as quickly as possible. The typical turnaround time is within 10 business days, but federal law allows up to 30 days to process a request.

Same-day, walk-in service is only granted in special cases if request can be accommodated. This is rare and should not be expected.

If I choose to receive my request with encrypted email. What does that mean?

If you are receiving the records through our encrypted email option, you will receive an email first with instructions outlining how to register.

What if I have a different type of request that is not listed above?
Email the Privacy Compliance officer at GFRD.Compliance@gilbertaz.gov for any other requests not listed above.
