Pre-Hospital Care Reports are protected under Federal and State law. To ensure that the Gilbert Fire & Rescue Department complies with its obligations a "Pre-Hospital Care Report Request” form must be filled out for each record requested along with the required information associated with your request. Below you will find the required form and documents needed to fulfill your request.

Same day walk in service is only granted in special cases if request can be accommodated. This is rare and should not be expected.
