1) Download one of the below PDF's (there are two forms: a fillable PDF and blank PDF if you wish to handwrite the actions you are taking);
2) List out the actions you are taking;
  a) Fillable PDF - electrically add details into the fields and print
  b) Blank PDF - print and handwrite business and action details
3) Sign and date the commitment letter;

Arizona Department of Economic Security (DES)

The Arizona Department of Economic Security (DES) will launch the Pandemic Unemployment Assistance System on May 12th.

If you haven’t already applied for Unemployment Insurance, please do so here, even if ineligible for regular unemployment.

Have specific questions? Contact the Unemployment Call Center at (877)600-2722. The Unemployment Insurance Call Center has extended its hours: Monday – Friday, 7:00AM to 6:00PM, and Sundays from 9:30AM to 5:00PM.

The SBA has resources to support businesses that are being affected by COVID-19.
