Guidelines & Conditions

  • All proclamation requests must be submitted at least 21 days prior to their request for issuance.
  • In order for the proclamation to be considered, this form must be completed in its entirety, and a draft of the proclamation must be emailed to Proclamations@GilbertAZ.gov. (additional instructions regarding the draft are provided below.)
  • Final approval of all proclamation requests will be given by the Mayor's office, and language may be subject to amendment.

How to Submit a Proclamation Request

Please complete this online form AND email a draft of your proclamation to proclamations@gilbertaz.gov.
